Independence

Homebird Care operates a Three Way Pathway to help each individual to move through a programme so they can experiment safely with increasing independence before moving on to complete independence should they have the ability to do so.

First Phase – This concentrates on:

• Building Relationships – Working through issues regarding social interaction.
  
• Being Empowered – Through peer group support.
• Identify your own needs – Positively and with the support of staff and to establish plans and methods to address these needs.
• Experience Positive Achievement – validated by their peers
  Community Integration.

Second Phase – Individuals will have the opportunity to move into a semi-independent setting where they can experiment with some of the practicalities involved in assuming greater personal responsibility, whilst retaining close staff support if required.

Third Phase – Individuals will have the experience of independent living and will assume high levels of responsibility for themselves.

Homebird Care will support individuals at the third phase to find suitable accommodation and will continue to support them until the individual does not require further support.
